Description des services:

Consists of an inter-professional team of physicians, nurse practitioners, social workers and dietitians who work in collaboration with other programs offered by Brock CHC to provide primary health care services to residents of Brock Township and vicinity * Services include:
- preventative health care
- treatment of chronic and episodic illnesses
- mental health and addictions counselling and referrals
- access to phlebotomy and electrocardiogram (ECG) services
